- A Nightmare on Elm Street (2010) [Dr. Gwen Holbrook]: Stabbed through the back of the head by Jackie Earle Haley, after Jackie suddenly appears in the mirror and reaches out from it, as Rooney Mara looks on in horror. (Thanks to Cody and Tommy)

- American Horror Story: Birth (2011) [Vivien Harmon]: Dies in childbirth; she later reappears as a ghost. (Thanks to Matt, Michael, and Chip)
- Nashville: Stand Beside Me (2017) [Rayna James]: Fatally Injured in a car accident when the police car she is in is crashed into by a pick up truck. She spends some time in the hospital in the next episode If Tomorrow Never Comes but soon dies of her injuries shortly afterwards with her family by her side.
